RPTKA / IMTA process may take 1.5–2 months.

Offshore means you are outside Indonesia or not allowed to enter until eVisa issuance.
Onshore means you are in Indonesia and must remain in the country until the process is completed.

Passport (+6 months validity) & personal photo
Permanent address in Bali
Bank statement (USD 2,000 if required)
Approved RPTKA / IMTA from Manpower Ministry
Additional documents as required by employer
Payment in advance

Overstay fine: IDR 1,000,000/day
